ACTIVE MEMBERSHIP shall be open to those individuals having responsibility in medical and/or provider activities. Active members shall pay dues and shall be eligible to vote and hold office. Active members shall be encouraged to join the National Association. Once you have completed the online application, you will be prompted to pay your membership dues ($50.00 for Active).
